Preparation For Your Appointment
If you would like to meet with someone from our company about the potential care for your child, please click the button below and schedule an appointment. Once we receive your requested appointment, our Office Administrator will contact you to gain more information and let you know if the time you picked is available.
If you receive an email response stating that we are able to speak with you either at your requested time or another time, please be advised that you will need to be prepared with certain information that we will obtain to determine if we are able to take over care of your child. If we contact you and you do not have this information available, then your appointment will be rescheduled to a later time. If at anytime you need to reschedule an appointment, please do so at least 48 hours in advance. Below is the list of things that we will need information on during your appointment.
-Child's Disability Diagnosis
-Name of current company you are receiving care from
-Name of child's Primary Care Physician who signs all authorizations
-Name of current DME company that you are using
-How many hours of care are needed per day?
